Understanding Lower Back Pain
Lower back pain (LBP) is one of the most common musculoskeletal problems affecting adults today. Globally, it is the leading cause of years lived with disability and affects people of all ages.
In the UK, around one in six adults are estimated to be living with back pain at any given time.
Although many episodes resolve with time, a significant number may recur or become more persistent—especially without proper care, guidance or lifestyle adjustment.

The Causes of MECHANICAL LOWER BACK PAIN
Mechanical lower back pain can stem from several different structures in and around the spine and therefore understanding the cause is key to providing the right treatment. Here are the four most common sources of lower back pain we regularly treat in clinic.
DISCOGENIC PAIN
Discogenic pain originates from the intervertebral discs — the soft, shock-absorbing cushions between the bones of your spine. Wear and tear, bulging, or herniation can irritate nearby nerves, leading to localised pain or symptoms like sciatica. This type of pain is often aggravated by sitting, bending, or lifting.
FACET JOINT IRRITATION
Facet joints are the small paired joints at the back of the spine that allow for smooth movement. Irritation or inflammation in these joints, often from poor posture or repetitive strain, can cause sharp or aching pain in the lower back, especially when arching or twisting the spine.
SACROILIAC JOINT IRRITATION
The sacroiliac (SI) joints connect the spine to the pelvis and play a key role in weight transfer between the upper body and legs. These joints can become inflamed or dysfunctional due to uneven loading, pregnancy, injury, or poor movement patterns. SI joint pain is often felt on one side of the lower back or buttock and may worsen when standing, walking, or climbing stairs.
MUSCULAR INJURY
Muscles in the lower back are responsible for supporting your spine and enabling movement. Strains or overuse injuries from lifting, sudden movement, or poor posture can lead to muscle tightness, spasms, or soreness that’s often worsened with activity but relieved by gentle stretching and rest.
